Registration Process
- Complete the registration form to the right.
- Select which Clinic you are registering.
- Complete all sections.
- Make sure your email address and mobile phone are correct.
- Once all information is entered, click on the register button.
- If successful, you will be redirected to a confirmation page.
- And, you will receive a confirmation notification via email.
- Assignors will review your registration. On or about January 5, 2026, approval notifications will be sent via email.
- Approved registrants will receive instruction and a payment link to complete your registration.
- Tuition fee for both clinics is $499.
