Registration Process

• Complete the registration form to the right.
• Select which Clinic you are registering.
• Complete all sections.
• Make sure your email address and mobile phone are correct.
• Once all information is entered, click on the register button.
• If successful, you will be redirected to a confirmation page.
• And, you will receive a confirmation notification via email.
• Assignors will review your registration. Approval notifications will be sent via email.
